After the initial sound effect of getting the item, the short fanfare loop, at a slightly slower tempo than normal, repeats, similar to how the fanfare repeats in-game if you don't press any buttons after the item message appears. At 4:18, "Africa" by Toto fades in and is mashed up with the original track.
This could be a reference to this popular Reddit post, where the poster claims that the advertised track sounds different on islands in the southern hemisphere, catches a fish and mashes up the resulting jingle with "Africa". The only difference is that here, "Africa" has been sped up to match the tempo of the advertised track, rather than the advertised track being slowed down.
